Yes you should. The way geofilter works is that when the game starts, it pings the servers and if it can't reach them, it will exclude them from matchmaking. If you don't restart the game you will have connection errors.

Great answer from Bert. 

You may not always need to, but generally for CoD it's a good idea to restart the game after making changes.

You can disable while in game to join a friend or something but if youre trying to disable then enable again or change anything yes it is best to restart
