There are only a couple of things that are going to cause you to have the lag you described.  At the times you are playing, your ISP could be overloaded with subscribers, you are joining a lobby with some players with very poor connections to the same server as you are on.  Their bad connection will cause you lag.  The server itself could have problems also. If you find yourself having laggy games you can temporarily ban those servers, move your home location to somewhere else, or reduce your geofilter range slider.

I have had very inconsistent games on COD WW2.  It's been similar to what you're describing.  Some games are amazing and some are really poor and laggy.. Nothing has changed with my home network or Netduma.

Try setting your radius to 600mi and set Ping Assist to 60ms. Your radius seems really small and you could probably make use of Ping Assist to find better games.

If you've added a huge amount of servers to the Deny list this could have reduced the amount of potentially good hosts in your area as well. What is your average ping to the game servers?

That's great. So if you click on the host of your game (the largest circle on the map) what is your ping to the host? Is that 15ms, or was 15ms your base ping?

Either way, it sounds like you've got a great connection for gaming.

Let us know later what your ping to the game servers is on average, and feel free to play around with the Geo-Filter. I'd recommend a 1000km distance, 0ms ping assist and strict mode on for local games. Keep Anti-Flood on 70/70 if that works for you since that's what we would recommend for gaming.

Also use the Reactive algorithm (in Congestion Control) and reset Device Prioritisation.
