How do i setup port forwarding for PC mw3

for example this is the TCP: 3074,3075,27015-27030,27036-27037 and UDP is UDP: 3074,4380,27000-27036 what do i use port or port range and do i select both udp and tcp

Well you've listed the protocols there so select TCP for the ones under that etc

Port is for 1 port that you don't make a range. You shouldn't need to use port forwarding but use these 3 rules:

(Port) UDP: 4380
(Range) TCP/UDP: 3074-3075
(Range) TCP/UDP: 27000 - 27037

You wouldn’t happen to have the battlenet ports would you?

According to the internet, the ports are UDP & TCP 4000, 6112-6119

No, I think you should use the CoD ports. Battle.net ports usually only apply to Blizzard's own games such as World of Warcraft and Starcraft.
