Hey, welcome to the forum!

 

To set a static IP for your PS4 take a look at the device manager for it's current IP then go to DHCP lease, click reserve and then enter the ending of it's IP and apply.

If you're manually port forwarding disable UPnP and disable UPnP forwarding in miscellaneous settings. If you have a blue box on the device manager enter the WAN IP into your ISP router's DMZ.
