How can this affect my gaming ?

It's very much dependent on how good your connection to the Google servers is - if it's not great then it doesn't necessarily mean anything bad for gaming. Though for the Ping Under Load test you should be able to improve the results by setting Congestion Control to always and experimenting with the percentages - change percentage, test, change test etc.

For ping under load better to change download or upload according to my test ?

Ping under load download ms should go up or down better ?  I have in pictures 19 ms , 18 ms and 20 ms 

also idle?

Both, you can see in the graph how they both fluctuate but looks like download needs to be adjusted more. Idle you can't really do much about and that is steady anyway

The download start with 20 then go high to 30 then go back to 20 it should be 20 from stat till end test in the graph mean same line not up down ??? To be better?

Ideally yes you want it to be as stable/consistent as possible with minimal increases overall
