My setup:

75 download 10 upload

Cable modem DOCSIS 3.0(bridge mode)

Linksys wrt 1900ac v2 (flashed with ddwrt)

Ddwrt setup:

My ISP has an Mtu of 1500 dropping it to 1472 is magic for me improved hit detection

TCP congestion control:
changing from Westwood to Vegas made my movement in game feel buttery smooth and my hit detection even better  Vegas works better with DOCSIS 3.0 cable modems I heard

TCP timeout 90secs

Upd timeout 90secs

I also reduced the TX queue length on my wan and lan to 2

These settings are simple and a game changer for me

I don't use QoS I have tried everything possible with QoS all placebo

My ISP apparently uses QoS on there end because my bufferbloat ratings are A+

I had fiber before but it didn't work well in my neighborhood so I switched back to my old cable provider which works well now due to less congestion on the lines because the majority of people in my neighborhood switched to fiber so I believe it eased up the traffic on the cable lines

Just wanted to share this after years of trying different methods and ISPs . But at the end if it all choose your ISP wisely switching back to cable from fiber made the biggest difference for me due to less people in my neighborhood using the cable ISP the ddwrt settings above just made it smooth and crisp

I'm from the Caribbean my lowest ping to the COD servers is 70ms even on 100 ping my games are crisp
